To report a new dissertation topic:
Fill in as much of the information as possible in the area above the line.
In the area below the line, provide only the "title," "degree" (your degree objective: Ph.D., D.M.A., etc.), "field" (the field in which you expect to receive your degree, e.g., musicology, music history and literature, music theory, etc.), "institution" (the full name of the institution from which you expect to receive your degree), "research director," "period or dates encompassed" (the period or approximate dates appropriate to the subject of your dissertation), and (if possible) at least a few additional index words. The remaining information will be supplied when the dissertation has been completed and accepted by your institution.
To report a completed dissertation:
In addition to the information requested for reporting a new topic, you should now (1) supply the date of degree; (2) provide details of binding, pagination, and the typographic form of your dissertation (choose one: typescript, computer print, or typeset); (3) check all the checkboxes that apply to your dissertation; (4) provide any additional index words; and, as available, (5) enter the Dissertation Abstracts, RILM Abstracts, and UMI numbers.
If your dissertation has been published (this will most commonly occur in the case of German dissertations), please supply a complete bibliographic citation, including ISBN, for the published version.
If you have not kept a copy of the information you supplied when reporting your dissertation topic and need a copy prior to reporting your completed dissertation, please contact the DDM Office and we will send you a copy of whatever information we have on file.
